Six-a-Side 2007 Disclaimer Forms
Could all teams that have received confirmation of their successful entry into this year’s six-a-side please make sure they download the linked disclaimer form.
It is a requirement of our insurance coverage that all players must have signed and handed in one of these forms before they will be allowed to play. There will be no exceptions to this rule, so print them out and get them signed before your first game of the season.
